I/we authorize and instruct any person, including but not limited to all local, state or federal government agencies and/ consumer reporting agencies, to complete and furnish Nation Wide Loan any information it may have or obtain in response to such credit, identity, income, or employment verification or fraud prevention inquiries. I authorize consumer reporting agencies to release to Nation Wide Loan my income and employment information in accordance with this paragraph, and consent to the periodic re-validation of this information prior to or during the term of my account relationship. All of the information referenced in this authorization, along with this application, shall remain the property of Nation Wide Loan, whether or not an account is opened.

My consent is for a one-time validation within the next 90 days.
